Poka Yoke - Fun to say; what does it mean?

Poka Yoke is an improvement method for decreasing human errors (Mistake Proofing) in processes, systems, and equipment.

"Ugh! I'm only human; I make mistakes. These cannot be prevented." We have heard this before. Is this acceptable?  Let's challenge this human error paradigm to help get things correct and complete the first time!


Through our day, mistakes occur; from Selecting the wrong items, Omitting information, Providing wrong information, and of course, the Mises: Mis-reading, Mis-measuring, Mis-adjusting, Mis-aligning. These mistakes are small yet can have a large impact.

  • According to PacketLabs, top cybersecurity breaches result from human error: email misdelivery, poor password hygiene, indequate/imcomplete data patching, & poor access control.
  • The National Academy of Medicine "To Err Is Human" report estimated that between 44,000 and 98,000 Americans die every year due to preventable adverse events.

A Mistake Proofing (or Poka Yoke) Device

Devices help redesign a process or piece of equipment so that it CANNOT be performed incorrectly. Consider all the devices for providing a credit card, or providing info with drop down options, or parts that only fit together one way. Here is another device we take for granted: 

Error: Electrical plug oriented backwards.

Defect: Reversed electrical polarity.

Device:  Make plug spades different width so reversed plug provides immediate feedback before inserting plug into outlet.
